15-Hour Surgery in Noida: Yatharth Hospital Successfully Performs Complex Liver-Kidney Transplant

Noida News (9 September 2026): Yatharth Hospital, Noida Extension, has successfully performed a highly complex Simultaneous Liver-Kidney Transplant (SLKT) on a 50-year-old patient suffering from end-stage kidney disease and chronic liver disease, marking a significant milestone in the hospital’s advanced transplant programme.

The 15–16-hour procedure involved a multidisciplinary team of liver and kidney transplant surgeons, anaesthesiologists, nephrologists, critical care specialists and blood bank professionals. The case presented several major clinical and surgical challenges, including an ABO-incompatible liver transplant and complex vascular anatomy during the kidney transplant.

Patient Had Multiple Serious Health Complications

The patient had been undergoing haemodialysis three times a week after developing chronic kidney disease following a right radical nephrectomy performed nearly a year ago for a kidney tumour.

His medical condition was further complicated by Type 2 diabetes, diabetic nephropathy and neuropathy, hypertension, single-vessel coronary artery disease and chronic liver disease caused by Hepatitis B infection.

Given the severity of both his kidney and liver conditions, the medical team determined that a simultaneous liver and kidney transplant was the appropriate treatment option.

ABO-Incompatible Liver Transplant Added to the Complexity

One of the major challenges in the procedure was the blood-group incompatibility between the patient and his liver donor.

The patient had an O-positive blood group, while his wife, who donated a portion of her liver, had a B-positive blood group, making the liver transplant ABO-incompatible.

The patient’s daughter donated one of her kidneys, with the kidney donor and recipient having compatible, matching blood groups. The transplant required extensive pre-operative planning and close coordination between multiple specialist teams to manage the complexity of the combined procedure.

Donor Kidney Had Two Renal Arteries

The kidney transplant presented another significant surgical challenge. The donor’s left kidney had two renal arteries, requiring complex vascular reconstruction to establish adequate blood flow to the transplanted organ. The surgical team used the Pantaloons anastomosis technique to reconstruct the renal arteries and facilitate their connection during transplantation.

Pre-operative CT angiography also revealed that the patient’s right iliac vessels were unsuitable for vascular anastomosis. Following detailed surgical planning, the reconstructed kidney was therefore connected to the patient’s left iliac vessels.

Both the liver and kidney donor procedures were performed using laparoscopic techniques, helping the surgical teams complete the donor operations through minimally invasive approaches.

Both Transplanted Organs Functioning Well

Following the 15–16-hour surgery, the patient showed encouraging signs of recovery. According to the hospital, both the transplanted liver and kidney are functioning well, with the patient receiving close post-operative monitoring and critical care.

Dr. Vishal Kumar Chaurasia, Group Director – Liver Transplant, GI & Robotic Surgery, said that simultaneous liver-kidney transplantation is among the most complex transplant procedures and requires extensive planning, specialised expertise and seamless coordination between several medical teams.

He said precise surgical planning and vascular reconstruction were crucial to establishing adequate blood flow to the transplanted kidney.

He noted that careful pre-transplant preparation, meticulous management during surgery and intensive post-operative monitoring played an important role in the outcome.
